1. Obviously the room was not dusted or surfaces cleaned and the bathroom was never mopped during our stay (a dirty mark on the bathroom floor from previous guests was never removed.

My non-5 star categories are: I would give Housekeeping: 2 to 3 stars, we kept having to call for extra towels, then the next day they would still leave only 2 towels for the 3 of us and we had to call again, they left dirty wine glasses and we had to call for clean ones, constantly calling but they always came quick with whatever we needed) Room service: food was excellent but ALWAYS took a full hour at least (even on the non-weekend mornings).
