2. I had a non-smoking room but you could tell the room had previously been a smoking room as it still had the smoke smell in it.

3. Some minor problems with the room: (a) found a silverfish in the bathroom soap dish (b) could not get a full set of non-feather pillows after asking twice and (c) the room had a faint mildew smell.

4. The carpeting appeared to be quite dated, and there was a smell that I think was some kind of cleaning supply, but it wasn't so strong that it didn't escape notice after a little while.
