5. My favorite two are Kona and Punta Mita, with the edge going to Punta Mita (based on preferences in cuisine, etc) -- but Kona is outstanding and possibly better in its own right.

7. When booking I would suggest talking to the hotel directly about your needs and they will do everything possible to accommodate them (I found better service ringing the resort than going through Global Reservations).
